You can register at the School of Sport & Exercise Sciences on
Wednesday 4 July      16:00 - 17:00     or
Thursday 5 July           From 08:30

Registration for this conference will be by invitation only and is restricted to 100 participants including the speakers.

To register please download the word file below. complete it and return it to Holly Etchell at h.etchell@bham.ac.uk

To return the form you can do one of the following:

1. Fill in the form, then save the Pdf file and attach this in an e-mail.
2. Fill in the form and click the submit button at the bottom right corner. You then choose the way you want to send the e-mail and the data will be automatically attached.
3. Fill in the form, print it and fax it to Holly Etchell (+441214144121).
